Test Centre

The Basics

You are responsible for submitting the completed Accommodated Test Request form to the Test Centre at least 3 working days before your test.

 

During exam week, completed forms must be submitted at least 7 days before your exam.

How to fill out the Test Centre Accommodation Form

Instructors

To facilitate the accommodated testing process:

  • Provide students with more than 3 days’ notice for upcoming tests. If the Accommodated Test Request form is not submitted with 3 days’ notice, the student may not get their accommodations. 
  • Tell the student how long the test will take. The student needs this information to complete the form and the Test Centre requires it for calculating extra time, booking proctors, etc.
  • Accept the Outlook invitation and complete the instructor portion of the invigilation form.
  • Provide the Test Centre with the test in advance of the scheduled test.

Students

To facilitate the accommodated testing process:

  • Fully complete the Accommodated Test Request form and submit to the Test Centre at least 3 working days before the test.
    • Note: 7 working days’ notice is required for exam week. If the Accommodated Test Request form is not submitted within the required timeframe, you may not receive your testing accommodations. In this situation, you will be expected to write with your class without accommodations.
  • If you are unsure of the test date or time, talk to your instructor.
  • Accept the Outlook invitation sent to your college email. The Test Centre does not use personal emails.
  • If you need to cancel your scheduled test, you must contact the Test Centre 24 hours in advance.
  • Adhere to the Student Honesty and Integrity policy and demonstrate academic integrity.
