Assignment Description

For this assignment you will:

  • Use the previous assessment and class material to craft an intervention plan for the assessment plan written in the previous assignment.

 

Requirements

  • Intervention plan (minimum 1000 words) which includes the following sections:

    • Analysis

    • Goals

  • APA format, reference list, and in-text citations

Note: In your assessment you did not need to use references. However, in this assignment, you will use references to support your work. You will need to find credible sources that are connected to the issues that you have identified (such as dementia or bipolar disorder).

Method

Using the assessment plan that you completed in the previous assignment, compose an intervention plan that includes the following sections:

Section One: Analysis (use Analysis as your heading)

  1. Begin by linking your intervention plan to your assessment.
    1. Review the strengths/challenges you outlined in your assessment and determine where and how best to intervene
    2. Use references to support your analysis – for example, what have you learned about the onset/definition, trajectory or treatment of the issues you have identified? How are you using that knowledge in your analysis?

 

Section Two: Goals (use Goals as your heading)

  1. Compose an intervention plan that includes the following:
    1. Goals: Using the SMART format outlined in your course (and program) resources, complete a minimum of four goals for the target (individual, couple, family etc.)
      1. For each goal, create a list of objectives that will lead the target to accomplishing the goal
      2. For each goal, identify at least one resource available in our community that could provide assistance in the target achieving their goal. Note how this resource would be of assistance.
  • Use references to support your goals/interventions. For example, find references that supports or contradicts the goal’s usefulness. That is, is there research that indicates that the goal will be useful in bringing about change? Is there research that indicates that different goals would be more helpful?
